The Integrated Community Health Coordinator will provide case management, care coordination and health education to individuals who are very low income and/or uninsured people of color residing in South Pinellas County with an identified health condition or who are in an at-risk category for a health condition.
Increase access to behavioral health and health services through linkage, technology (i.e., telehealth), housing and health care provided by comprehensive care connections.
Plan and develop CARE concepts with community partners to identify specific results, indicators and strategies that are aligned with our combined mission.
Bachelor's Degree or High School diploma plus 4 years' experience.
Possess initiative, strong organizational and communications skills and the ability to work independently.
