Position Purpose:
The Pharmacist provides comprehensive pharmaceutical care within the New Mexico VA Health Care System, ensuring high-quality medication management services that meet or exceed national standards established by The Joint Commission (TJC) and VA regulations. This position plays a crucial role in delivering top-tier pharmacy services to veterans, ensuring patient safety, regulatory compliance, and the highest standard of pharmaceutical care.
Duties and Responsibilities:
- Dispense prescribed medications and provide detailed information to patients regarding their proper use.
- Advise healthcare providers on selection of medication, dosage, interactions, and potential side effects.
- Review and evaluate medication orders for therapeutic duplications, incompatibilities, drug interactions, allergies, and side effects.
- Identify and resolve potential medication-related issues such as inappropriate dosing or incorrect strength, collaborating with providers to recommend formulary-compliant alternatives.
- Counsel patients on prescription medications, including possible adverse effects and interactions, as well as durable medical equipment and home healthcare supplies.
- Maintain positive interactions with healthcare providers, patients, and pharmacy staff to foster a collaborative work environment.
- Enter and verify all medication orders in the electronic patient medical record system (VISTA).
- Ensure strict compliance with federal and state regulations regarding controlled substances, including accurate record-keeping and perpetual inventory control per VHA Handbook 1108.01.
- Handle hazardous drugs and sterile compounding in compliance with USP 800 regulations.
- Demonstrate proficiency in aseptic techniques and sterile product compounding in accordance with USP 797, completing all required facility competencies and testing before engaging in sterile compounding activities.
Qualifications
Requirements:
- Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited college or school of pharmacy with a Bachelor of Science in Pharmacy (BS Pharmacy) and/or Doctor of Pharmacy (Pharm.D.) degree.
- Hold an active, full, and unrestricted pharmacist license in any U.S. State, Territory, or Commonwealth, or the District of Columbia.
- Must be licensed for controlled substances and qualified to practice in any state.
- Completion of all VA educational and mandatory training requirements, including ACLS, BLS, and CPRS.
- Prior to starting work, pharmacists must complete all necessary background checks, fingerprinting, TMS training, PIV sponsorship, and obtain authorization within the VA computer system (VISTA) to provide patient care.
Working Conditions and Environment
This position is on-site in a medical facility. Driving may be required.
Physical Requirements
The position requires repetitive motion, such as standing, walking, sitting and bending. Light lifting, up to 20 pounds, may be required.
