Nexus-PATH is located across North Dakota and offers mental health services, outpatient clinical services, and treatment foster care. We have an exciting opportunity to hire a full-time Foster Care Referral Manager in our Fargo, ND Nexus-PATH office!   

 

Schedule/Pay/Location: 

  • On-site in Fargo, ND
  • Some flexibility in scheduling based upon client needs 
  • Monday-Friday work schedule
  • $52,000-$58,000 starting salary

Primary responsibilities:

  • Provides the first line of contact for referrals for our foster care programs.
  • Responsible for entering all documentation on referrals into Evolv.
  • Completes a systemic assessment of the needs of the client and family.
  • Interviews client either in person or virtually to get to know their strengths and areas of concern.
  • Collaborates with referring worker on family finding activities.
  • Provides consultation to referral source when necessary to meet the client’s needs.
  • Responsible for handling of internal referrals (Client/s placed within agency home and needing to be transferred).
  • Attends Child and Family Team meetings for client on the referral list.
  • Attends Regional and State Review Teams meetings as requested.
  • Attends internal agency meetings to match with potential homes.
  • Routinely screens for trauma exposure and related symptoms and demonstrates competence in using screening tool results to enhance service provision.
  • Routinely makes resources available and shares knowledge with foster families and providers on trauma exposure, its impact, and trauma-focused treatment.
  • Engages in efforts to strengthen the resilience and protective factors of children and families impacted by and vulnerable to trauma.
  • Integrates trauma-informed care principles into practice by sharing knowledge and understanding of trauma with the client, foster family, and relevant team members. Additionally, utilizes and advocates for the use of trauma-sensitive strategies to support the client/s and their family.
  • Recognize and value cultural differences in all aspects of work and service delivery.
  • Responsible for assuring that Matching, Intake and Admissions functions related to state licensure and accreditation are implemented consistently statewide and are in accordance with applicable rules/standards.
  • Complies with local office procedure for referral and service delivery.
  • Remains current on all required Nexus training requirements and maintain a thorough working knowledge of the organization's policies and procedures.
  • Provides complete and required documentation, within established timelines relating to case management.
  • Leadership & Oversight
  • Plan, organize, and effectively manage multiple priorities and perform activities in a manner that makes the best use of existing resources, staff, and time.
  • Participates in or leads staff and supervision meetings, and required in-service meetings
  • Provides timely and accurate communication to supervisory and management staff and team members regarding significant events, to ensure continuity of care and safety of clients
  • Manages the caseload or workload assignment of direct reports
  • Takes on or delegates necessary casework in assigned staff’s absence
  • Actively participates in the interviewing and hiring process; orients, mentors, and coaches staff while continuously assessing, developing enhancing employee knowledge and skills.  
  • Exhibits clear, professional, respectful, and effective communication when interfacing with clients, coworkers, families, and external contacts; willingly shares knowledge, information, and resources appropriately.


Required Qualifications: 

  • Bachelor’s degree in a Human Services discipline 
  • 1+ year experience in community work and care coordination experience required
  • Ability to work as an intrinsically motivated self-starter with ability to prioritize work, manage time effectively, and add value to the role at all times
  • Proven history of handling difficult cases with excellent results
  • Must be open to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values.
  • Valid driver’s license required. Must meet state regulating agency and Nexus Home Office driving requirements


Preferred Qualifications: 

  • 3+ years of experience in community care work and coordination

Travel: 

Use of personal vehicle for regular local business travel will be required to support the clients and agency business offices; travel to company-sponsored events may be required. 


Work Environment: 

This job operates in a professional office and community environment that may include homes, shelters, group homes, and crisis settings. This role routinely uses standard office equipment such as desktop and/or laptop computers, photocopiers, and smartphones.

While performing the duties of this job, the employee is regularly required to talk and hear. Must be able to sit in a car, at a desk, and computer for extended periods; have visual acuity conducive to extensive computer monitor work; able to twist, bend, and stretch in a manner conducive to the execution of daily activities within an office environment. May involve repetitive wrist motion. Maybe required to lift up to 50 pounds unassisted and up to 100+ pounds with assistance. Expected Hours of Work:

Days and hours of work are Monday through Friday, with the expectation to work specific schedules and hours as identified and communicated by management. Occasional evening and weekend work may be required as job duties demand.
