About the Position 

We are looking for an Intensive Case Management (ICM) Therapist to join our team on-site in Medford, Oregon! The ICM Therapist will provide strength-based, client-centered, solution-focused and motivational enhancement clinical services to include therapy, case management, and skills training in various community settings to clients with severe and persistent mental illness under Jackson Care Connect. They will conduct mental status examinations and complete and update risk assessments and safety plans in a timely manner. The ICM Therapist will complete post-hospital follow-ups within 7 days of client discharge from the behavioral health unit and refer clients to appropriate agencies for ongoing care. 

 

Work Schedule: Monday through Friday, 8:00am – 5:00pm (Full Time Day)

 

What You’ll Make

$6,000-$7,250 per month DOE/Credentials

Additional 5% Language Differential offered for Bilingual or Multilingual candidates.

Pay Scale

Unlicensed: $6,000 - $6,750 per month Professional Oregon Mental Health License: $6,750 - $7,250 per month 

 

About the Program

The ICM team provides community-based services that meet clients where they are – literally. Through a strength-based care lens and team approach, we provide intensive support which is individualized to meet your immediate therapeutic and case management needs. Upon approved referral, your ICM team will include a therapist, case manager, and peer support specialist who work alongside you to identify and enhance individual strengths, build and maintain connections to community support systems, learn and apply relevant skills, increase self-sufficiency, and foster sustainable change for your overall health and well-being.

A primary goal of the ICM team is to help divert individuals from hospitalization when appropriate and/or step them down from higher levels of care into available sub-acute or community-based treatment

 

ColumbiaCare Services is a qualifying employer for the Public Service Loan Forgiveness (PSLF) Program. 

 

What You’ll Need

 

Must QMHP certified through Mental Health & Addictions Certification Board of Oregon (MHACBO) or meet eligibility qualifications to register with MHACBO. Eligibility requires:

    Master's Degree in behavioral science field or a bachelor’s degree in Nursing and be licensed by an Oregon licensing boardExperience performing mental health assessments, diagnosis, treatment plans and working with SPMI population is preferred.

 

If not already registered as a QMHP through the Mental Health & Addiction Certification Board of Oregon (MHACBO), we will assist you in registering within the first week of employment. 

 

Driving is an essential function of this position. Successful applicants must provide a valid driver's license from their state of residence or obtain and submit one within 30 days of hire, maintain a safe driving record, and have the ability to pass a DHS criminal background check and a pre-employment drug screen.

 

This position requires the ability to frequently talk, listen, use hands and fingers, reach, stand, walk, sit, bend, stoop, climb stairs, as well as lift, carry, push, and pull up to 40 pounds. It may require the ability to occasionally use ladders, squat/kneel, and perform other physical tasks as applicable. In addition, you must successfully complete crisis de-escalation and CPR trainings, which require performing various physical, hands-on maneuvers and techniques and may require an individual to lift and transfer clients.

 

Per OAR 309-035-0135, Direct Care staff must be 18 or older at the time of hire.
