Health & Wellness Outreach Manager

Minimum Education and Experience Requirements: A high school diploma or GED coupled with a minimum of seven years of professional experience; or an associate degree with relevant professional experience; or a bachelor's degree accompanied by at least three years of professional experience.
Preferred Qualifications: Certification as a peer support specialist (state or national credential) and a minimum of one year of prior experience in peer support work.
Job Summary
Facilitates multiple enrichment program initiatives with a focus on health and physical activity for the University and local community. Serves as a community liaison for regional and national programs and develops partnerships with the local community centers and University Community Engagement Centers. Manages contracts and vendors; performs budgetary functions.
Essential Functions
- Develop and collaborate with community partners;
- Delivering manualized health education information to program participants;
- Helping participants identify appropriate health and social services;
- Provide participants with social support and tangible assistance to overcome barriers to care access (e.g., appointment reminders, accompanying an individual to a doctor’s visit);
- Meet with participants in person and/or virtually based on individual needs and preferences;
- Maintain up-to-date and accurate logs and documentation of services provided and supported in electronic entry systems (e.g., REDCap);
- Participate in supervision and project and team activities.
Physical Effort
Must possess the ability to navigate between campus buildings independently and engage in physical activities within the community.
