A-Team Solutions is seeking (15) Licensed Clinical Social Worker (LCSW) for the VA of Greater Los Angeles to support our homeless-Veterans at off-site community locations throughout the Greater Los Angeles area. Coverage will start ASAP. Normal working hours are 8:00 am to 4:30 pm, Monday through Friday some of which will be at VAGLAHS, others offsite. LCSW services will be directed to the needs of patients confronted with crisis, homelessness, substance-abuse, life-threatening illnesses, traumatic losses, and chronic diseases or disabilities including chronic mental illness. This includes those social and environmental problems which represent barriers to effective use of treatment, or which prevent or impede the Patient?s successful, healthy living. Local candidates strongly preferred.

? Provide housing-search assistance by locating suitable housing in the community and at project based voucher locations and helping maintain an inventory of safe, available units accepting rental subsidies; assisting Veterans with completion and submission of rental applications and credit reports; communicating with landlords to secure and maintain housing; conducting outreach to prospective landlords and property management groups on behalf of homeless Veterans, and educating individuals and organizations on the benefits of leasing to Veterans; meeting Veterans at housing units both for viewing and walk-through purposes; assisting Veterans in obtaining move-in and security deposit, as well as furniture and household items; facilitating individual and group housing searches; creating outreach materials for landlords; participating in Public Housing Authority (PHA) sponsored landlord outreach and appreciation events.
? Provide and document housing-related advice to patients via face to face or telephone contact using required documentation systems including the Computerized Patient Record System (CPRS) and the Homeless Operational Management Electronic System (HOMES) and/or other required centralized documentation systems that may be developed during the course of the contract.
? Participate in scheduled meetings to review and evaluate the services provided to our Veterans, identify opportunities to improve the services delivered, and recommend corrective action when problems exist.
? Coordinate with other stakeholders in the VAGLAHS CERS departments including: Transitional Housing, Community Resource and Referral Center (CRRC), Domiciliary, Employment Services, Quality Management and the HPACT Clinic.
? Comply with VA and VAGLAHS information and security policy and procedures.
? Utilize clinical judgment in the assessment of patients? problems and strengths.
? Complete assessments and other documentation as required, including psychosocial assessments and assessments as needed per program regulations.
? Establish and maintain professional relationships with patients and significant others.
? Develop, coordinate, and carry out treatment plans, as appropriate, under the direction of the designated social work lead.
? Document and report suspected cases of abuse within the required timeframe and in conjunction with a discussion with the designated social work lead.
? Mobilize VA and community resources on behalf of Veterans and informs Veterans and/or their families about these resources.
? Interview patients/families to assess resources needed in the areas of housing, financial assistance, extended-Care facility placement, transportation, home help, support groups, respite, day healthcare, day treatment programs, legal aid, and other services.
? Complete clinical reminders, encounters, and progress notes at each interaction with the Patient.
? Complete the Suicide Risk assessment for at-risk patients.
? Complete/close all assigned consults.
? Under the direction of the designated social work lead, coordinate and oversee the process for placement into community nursing homes, rehabilitation programs, substance-abuse programs, emergency shelters, board and Care facilities, homeless transitional-housing programs, the Domiciliary, and other various VA and community programs.
? Assist Veterans with maintaining permanent housing at the individual desired location.
? Assist patients and families with identified psychosocial needs, Patient/family resistance, or complex problems.
? Communicate verbally and in writing with members of the interdisciplinary team regarding the Patient?s plans and resource linkages.
? Work with community resources to clarify and expand their services for VAGLAHS patients/families.
? Maintain and update resource files in assigned area of community resource needs such as transportation, meal programs, housing/shelters, adult day Care, support groups, Department of Public Social Services, extended Care facilities, and other resources as required by VAGLAHS.
? Document Patient-Care activities in VAGLAHS Medical Records in accordance with Departmental standards and VA policies.
? Perform other ancillary duties as requested by the designated social work lead and/or the CERS Clinical or Administrative Directors.
? Communicate with staff at other VA facilities and community agencies. With the consent of the Veteran, provide needed medical/psychosocial information to support referrals for continuity of Care.
? Share information about resources with the team and other CERS and Medical Center Staff.
? Comply with the VAGLAHS Center and Care Line directives, procedures, policies, and statistical requirements.
? Provide emergency-room, social work coverage, and assistance with transplant assessments, when requested.
? Complete incident reports, inquires/controlled correspondences, and reports when requested and with the support and guidance of the designated social work lead.
? Participate in VA stand-downs during regular assigned tour of duty, when requested.

? Licensed Clinical Social Workers must be licensed through the State of California?s Board of Behavioral Sciences (BBS) at the independent licensure level. License must be current and unencumbered. Proof must be provided prior to their first assignment, and at their licensure renewal date. This also applies to any other required certificates.
? Must have copy of MSW degree from a Social Work School accredited by the Council on Social Work Education.
? Ability to clearly write and converse in English, using correct grammar and enunciation.
? Ability to use computers for basic word processing functions, e.g., Microsoft Office, Microsoft Excel.
? Knowledge of the State of California Welfare Programs.
? Understanding of human behavior and the impact of the illness on the Patient and family.
? Working knowledge base of medical/psychiatric diagnostic treatment, and procedural terminology.
? Ability to relate to individuals from different cultural and socioeconomic backgrounds within and outside VAGLAHS.
? Basic understanding of VAGLAHS social service role, housing-first model of housing, harm reduction, and homeless services within the VAGLAHS catchment area.
? Ability to organize and maintain extensive community resources.
? Skill in prioritizing assignments to assure the completion of work in a timely manner when there are deadlines, despite a heavy workload.
? Skill in working independently on assignments under frequent interruptions or distractions.
? Ability to establish and maintain cooperative working relationships with personnel in CERS Department, personnel in other departments though the medical center, and people from outside of the VAGLAHS.
? Ability to relate to patients, families, staff and community groups in a sensitive and humanistic manner.
? Must possess a current, valid, and unencumbered California Driver?s license if assigned to programs where it is necessary to drive in order to provide services to patients in the community
