We are seeking a skilled and compassionate Respiratory Care Navigator to support pediatric and/or ALS ventilator-dependent patients in the home setting. This is a highly autonomous, relationship-driven role where you become the trusted clinical expert guiding families through complex respiratory care outside of the hospital.

What Youll Do

As a Respiratory Care Navigator, you will:

• Manage and monitor a dedicated caseload of ventilator-dependent patients

• Perform comprehensive clinical assessments and ongoing reassessments

• Provide in-depth education and hands-on training to patients and caregivers

• Set up, monitor, and troubleshoot advanced ventilator and respiratory equipment

• Serve as the primary clinical liaison between families, physicians, and interdisciplinary teams

• Ensure safe equipment use and respiratory stability in the home

• Maintain timely, accurate documentation in compliance with regulatory standards

You will play a critical role in preventing hospital readmissions and empowering families with confidence and clinical knowledge.

Schedule & Work Environment

• Full-time | MondayFriday | 8:30 AM 5:00 PM

• Work performed in patient homes and community settings

• Frequent travel within assigned territory

• Independent field-based role with strong clinical support

What You Bring

Required:

• Active, unrestricted RRT or RN license

• Pediatric and/or ALS ventilation experience

• Tracheostomy care experience

• Valid drivers license with clean driving record

• Willingness to travel within territory

Preferred:

• Home health or home respiratory care experience

• Hospital respiratory or critical care background

• DME/HME experience
