Responsibilities:

Provide one-on-one care to patients in their homes, focusing on their physical, emotional, and social needs
Assist with daily living activities, such as bathing, dressing, and grooming
Support patients with medication management, meal preparation, and light housekeeping tasks
Encourage independence and engage patients in activities that promote physical and mental well-being
Collaborate with our interdisciplinary team to develop personalized care plans and ensure seamless transitions
