• Shift Duration: 8-hour shifts
• Schedule: Monday through Friday, approximately 8:00 AM to 4:30 PM
• On-Call/Weekend Requirement: None
• Mileage: Reimbursed between patient visits
• Estimated Weekly Pay: $2,030-$2,190

Job Requirements

• Bachelor's degree in Physical Therapy or a related field
• Current Physical Therapist license
• CPR certification required
• At least 1 year of Physical Therapy experience preferred, ideally in home health, outpatient, or rehab
• Home health documentation experience preferred; HCHB experience is a plus
• Strong communication, organization, and time-management skills
• Ability to work independently while delivering high-quality patient care

Responsibilities

• Provide skilled Physical Therapy services to patients in a home health setting
• Develop and implement individualized treatment plans based on patient goals and needs
• Assess patient progress and adjust care plans as appropriate
• Educate patients and caregivers on therapy goals, exercises, and safe mobility techniques
• Document evaluations, treatments, and patient outcomes accurately and timely
• Coordinate care with physicians, nurses, and the broader clinical team to support positive patient outcomes
• Travel between patient visits within the assigned coverage area in Easton and surrounding Maryland communities
