Job Summary
Synergy Homecare MSP Metro (office in Roseville) is seeking an experienced Registered Nurse to provide care in clients homes throughout the central metro area. This is a flexible part-time position (approximately 16-20 hours/week).

Duties:
Assess clients in their homes, create Plans of Care, and provide nursing services
Assist with caregiver training/education
Collaborate with clients, family members, care-providers, and co-workers to provide individualized client care
Provide reliable care and complete timely documentation of all visits and assessments
Qualifications:
Nursing degree, along with a valid MN registered nursing license
Strong clinical skills for assessing client needs and providing care
Communication skills
Critical thinking skills
Compassion and empathy
Customer service skills
Computer skills
Valid Drivers license, insurance and a reliable vehicle
