We are seeking compassionate and skilled Caregivers Needed to join our team in Manchester, New Hampshire, United States. As a Caregiver with SYNERGY HomeCare, you will have the opportunity to make a positive impact on the lives of our clients and their families.

Responsibilities:

  • Provide high-quality, compassionate care to clients in their homes, assisting with daily living activities and personal care needs

  • Develop and implement individualized care plans to meet the unique needs and goals of each client

  • Work collaboratively with clients, family members, and other healthcare professionals to ensure seamless care coordination

  • Maintain accurate and detailed records of client care and progress

  • Participate in ongoing training and education to stay up-to-date on best practices and industry developments

Requirements:

  • Caregiver experience preferred, but not required

  • Ability to work with minimal supervision and make sound judgments when necessary

  • Strong communication and interpersonal skills

  • Ability to lift, transfer, and move clients as needed

  • Reliable transportation and a valid driver's license (if required for client visits)
