Job Summary

We are seeking an RN Case Manager - Remote for a temp-to-hire opportunity supporting Medication Therapy Management and Disease Management programs in San Juan, PR. This role is ideal for a compassionate Registered Nurse who enjoys patient education, chronic disease management, medication safety, and meaningful beneficiary outreach.

In this role, you will work in a collaborative healthcare services environment with supportive leadership, multidisciplinary teamwork, and a strong focus on improving health outcomes. This is a great opportunity to use your clinical knowledge in a case management setting while helping beneficiaries better understand their medications, treatment options, and chronic conditions.

Key Responsibilities

- Conduct phone and face-to-face outreach with beneficiaries enrolled in MTM and Disease Management programs.
- Review and update medical histories, chronic conditions, medication treatments, and lifestyle information.
- Educate beneficiaries on medication adherence, proper medication use, treatment options, and chronic disease management.
- Answer questions regarding medication therapies, bioequivalent alternatives, and available care options.
- Collaborate with Health Educators and cross-functional teams to support clinical projects and beneficiary care coordination.
- Ensure program activities align with Centers for Medicare & Medicaid Services requirements.

Compensation and Benefits

- Pay rate: $21 per hour.
- Job type: Temp-to-hire.
- Location: San Juan, PR.
- Work model: Flexible hybrid schedule with required on-site workdays in Puerto Rico.
- Schedule may vary based on business needs.
- May require on-call availability, including evenings, weekends, and holidays, depending on operational needs.
