Job Details

• Setting: Home Health
• Employment Type: Travel Physical Therapist
• Location: Fort Worth, TX 76107
• Schedule: Monday through Friday, approximately 8:00 AM to 4:30 PM
• Shift Duration: 8 hours
• Hours per Week: 40
• Weekend Requirement: 1-2 weekends per month
• Estimated Weekly Pay: $2,030 - $2,190
• Duration: 15 weeks
• Start Date: 9/28/2026

Physical Therapist Job Requirements

• Bachelors degree in Physical Therapy
• Current Physical Therapy license
• BLS/CPR certification
• Valid drivers license
• At least 1 year of relevant Physical Therapy experience preferred
• Home health experience strongly preferred
• Proficiency with HCHB preferred

Responsibilities

• Perform start-of-care (SOC), resumption-of-care (ROC), evaluations, and treatment visits
• Develop and implement patient-centered therapy plans in the home health setting
• Assess patient progress and adjust treatment plans as needed
• Document care accurately and in a timely manner in the EMR
• Travel throughout the assigned service area to provide consistent patient care
• Maintain strong communication with patients, families, and the care team
• Support quality outcomes through compassionate, skilled rehabilitation services
