Job Requirements

• Active Physical Therapist (PT) license or eligibility to practice in Wisconsin
• At least 12 months of home health experience within the past 3 years
• Minimum 1 year of relevant clinical experience required
• Experience working with home health documentation and patient care plans
• Familiarity with HCHB preferred
• Strong communication, organization, and time-management skills

Responsibilities

• Evaluate patients in the home setting and develop individualized treatment plans
• Provide skilled physical therapy interventions to support mobility, safety, and functional progress
• Track patient outcomes and adjust care plans as needed
• Educate patients and caregivers on therapy goals, exercises, and home safety strategies
• Document visits accurately and timely in the EMR system
• Collaborate with the care team to help ensure coordinated, high-quality patient care
