Job Requirements

• Minimum of 12 months of home health occupational therapy experience within the past 3 years
• Active Occupational Therapy license or eligibility in Wisconsin
• Experience with Homecare Homebase (HCHB) preferred
• Strong clinical judgment, communication skills, and independent time management
• Ability to travel within the assigned territory as needed

Responsibilities

• Provide skilled occupational therapy services to patients in their homes
• Evaluate patient needs and develop individualized treatment plans
• Support patients in improving mobility, activities of daily living, and functional independence
• Document visits, treatment plans, and progress accurately and promptly
• Collaborate with the interdisciplinary care team to support quality patient outcomes
• Deliver compassionate care while maintaining a safe and efficient workflow
