Job Summary

Works in collaboration with the MDS coordinator and interdisciplinary team to assess, monitor and evaluate services necessary to meet individual resident needs.


Essential Functions

  • Assists with establishing and ensuring a viable treatment plan that will provide high quality, cost effective outcomes.
  • Leads team to assess, monitor, and evaluate services necessary to meet individual needs with the established RAI process.
  • Analyzes variances that occur in the plan of care and follows the RAI process with implemented outcome based plan to maximize residents optimal medical/ functional level.
  • Assists with coordination of efficient discharge planning to ensure recovery through the continuum of care via long term care, community and home health services.
  • Ensures the MDS assessment and documentation is completed in accordance with the current applicable federal and state guidelines and regulations and as directed by the Director Of Nursing and the facility policy.
  • Ensures medical record documentation related to the RAI process reflects current resident status.
  • Collects and distributes required outcome data.
  • Maintains responsibility for clinical application of the RAI/MDS process.
  • Responds and implements resolutions to resident, family, and staff concerns.

Qualifications
  • Required Associate's Degree Associates degree or higher in Nursing
  • Preferred Bachelor's Degree in nursing
  • 3 years of relevant experience 3 years experience in nursing preferably in long-term care Required
  • LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required
