If that sounds like an organization you want to be a part of, we would love to have you.ROLE OVERVIEW and PURPOSEThe Physical Therapist Assistant works to support the mission and vision of Neighborhood Healthcare (NHC) by supporting participants rehabilitative physical therapy (PT) treatment.This role will assist physical therapist with participant evaluations and treatment plans for program participants.In addition, this role will assist participants to remain independent and live safely in the community.RESPONSIBILITIESAssists physical therapist implement PT programs for participants accessing day services, including therapeutic exercises, neuromuscular reeducation, rehabilitation services, functional activities, gait trainings, orthotics trainings, prosthesis, and trainingsMakes recommendations to the physical therapist to modify the program based on the patient population, as neededPerforms assessments of patients to determine abilitiesAssists physical therapist implement individualized plans of care to decrease or eliminate disabilities, increase or maintain capabilities for independence, and develops functions to a maximum levelEnsures individual plans of care are followed accuratelyProvides a variety of physical therapies aligned to participants plans of care, including functional ability tests, treatment to relieve pain, and develop or restore functionAssists participants to achieve and maintain maximum performance using physical means, such as exercise, heat, sound, water, light, or electricityGuides patients and caregivers in the use of adaptive devices and therapeutic, creative, and self-care activities for improving functionEvaluates the effectiveness of treatments and makes recommendations to modify goals, as neededRecords and signs treatments in the patient's health recordEnsures services provided are in the clinic are conducive to restoring dignity and self-esteem to all patientsAssists physical therapists to ensure that required equipment and supplies for the physical therapy program are available and/or ordered, as neededKeeps abreast of all regulations and standards to ensure compliance with orders or directives issued by duly constituted government and regulatory agencies (PACE, CMS, DHCS)Participates in training programs for staff and caregiversEDUCATION/EXPERIENCEAssociate degree in physical therapy assisting required; an emphasis on geriatric populations is preferredCalifornia Physical Therapy Assistant license in good standing is requiredOne year of physical therapy assistant experience within an aged and disable environment is preferredBilingual (English/Spanish) is requiredADDITIONAL QUALIFICATIONS (Knowledge, Skills and Abilities)Excellent verbal and written communication skills, including superior composition, typing and proofreading skillsAbility to interpret a variety of instructions in written, oral, diagram, or schedule formKnowledgeable about and experience with principles and methods for treatment and rehabilitation of disabilities and functional impairmentsAbility to use ingenuity and imagination in adapting modalities to individual needsKnowledgeable and understanding of Older Americans Act, Americans with Disabilities Act, and Area Agencies on AgingAbility to establish and maintain therapeutic relationships and to participate in a multi-cultural, multidisciplinary team. and strong interpersonal skillsAbility to successfully manage multiple tasks simultaneouslyExcellent planning and organizational abilityAbility to work as part of a team as well as independentlyAbility to work with highly confidential information in a professional and ethical mannerPhysical RequirementsAbility to lift/carry 50 lbs/weightAbility to stand for long periods of timeNeighborhood Healthcare offers a generous benefit plan that includes:Partially company paid Medical, Dental, and Vision Plans.
