Legacy is recruiting an Advanced Practitioner for our Mt. Hood clinic, located at 24988 SE Stark St Medical Office Bldg 3, Suite 220, Gresham, Oregon. Our clinics support local communities by providing services to a variety of patient populations. With more than 25 clinics serving the Portland/SW Washington surrounding communities, and rural locations such as Silverton, Keizer, Sandy and St. Helens, Legacy is helping keep the Pacific Northwest healthy.

Our clinics have well-supported teams to include panel coordinator, social worker, RN case manager, behaviorist, pharmacist as well as an individually assigned MA's.

Responsibilities

  • Responsible for the patient throughout the course of treatment, including reviewing clinical history, providing the physical examination, making the diagnosis, and treating both chronic and episodic disorders.
  • Work in collaboration with the team and seek the advice of a team physician whenever a case falls outside the scope of expertise.
  • Maintain own panel of patients and share call coverage for the clinic.

Qualification and Licensure Requirements.

  • National certification as a Nurse Practitioner (NP) or Physician Assistant required.
  • Current applicable state license as a Nurse Practitioner with Prescriptive Privileges (NP-PP) or Physician Assistant required.
  • Unrestricted Oregon Medical license.
  • Current BLS certification from the American Heart Association
  • Follows guidelines set forth in Legacy’s Values in Action.
