If you think this is something you may have interest in, please feel free to apply or contact one of our Concordia locations for more details!Qualifications:Licensed as a Physical Therapist in the Commonwealth of PennsylvaniaLicensed as a Physical Therapist Assistant in the Commonwealth of PennsylvaniaLicensed/registered as an Occupational Therapist in the Commonwealth of PennsylvaniaCertified as an Occupational Therapy Assistant in the Commonwealth of PennsylvaniaEssential Duties and ResponsibilitiesDevelopment and modification of individualized, goal-directed treatment plansProvides therapeutic actions to relieve pain and/or develop or restore/regain maximum independenceImplements treatment plans, monitors resident response to intervention, and modifies treatment to attain goals in accordance with physician orders, plan of care, and discharge plansDevelops appropriate home or community programming to maintain and enhance the performance of the resident in their own environmentDocuments results of assessment, treatment, follow-up, discharge, and billing notes in an accurate, timely manner in accordance with Standards of Practice, state and federal regulations, and policies/proceduresCommits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards and Concordia policies and procedures. Maintains confidentiality of resident and employee records and information. Has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to a supervisor/manager or to the Corporate Compliance Officer.
