At Crestwood Medical Center, we believe in the power of people to create great care!  We look forward to you becoming the next RN Case Manager, Monday - Friday with some weekends 8 hour shifts at our 180 bed full service acute care facility. As a Registered Nurse Care Manager you will be responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management, and developing a safe appropriate discharge plan in collaboration with the multidisciplinary team. You will facilitate the progression and transition of care using established criteria in conjunction with the multidisciplinary team. You will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.

Requirements

Required Education: Associate Degree in Nursing or Nursing Diploma
Preferred Education: Bachelor’s Degree in Nursing

Required Experience: 2+ years’ experience in Care Management or 3+ years’ experience in clinical nursing
Preferred Experience: InterQual or MCG experience

Required License/Registration/Certification: Currently licensed as a Registered Nurse in Alabama
Preferred License/Registration/Certification: ACM or CCM Certification
