Job Summary:
The Hospital Case Manager coordinates patient care for Hospital Services and Therapeutic Foster Care from initial assessment through discharge, acting as a liaison between the patient, patient's family, and other professionals and agencies. They are responsible for preparing treatment plans and discharge portions of treatment plans coordinating actions of the multi-disciplinary treatment team members. They coordinate treatment conference, coordinate transfer to and from the facility, and coordinate outpatient appointments after discharge.

Minimum Qualifications:
Bachelors in Social Work. Licensed Social Worker preferred.
