As a Connection Leader at Pathfinder, you will play a crucial role in managing the day-to-day services in Medicaid Waiver funded settings while providing supervision and support to Direct Support Professionals (DSP) working in these settings. Your mission will be to act as a mentor, teacher, and advocate for individuals with intellectual and developmental disabilities, helping them to lead fulfilling lives as active members of their communities.

Responsibilities include overseeing the operation of assigned service locations to ensure that the services provided meet the expectations of the individuals served and comply with governing oversight requirements. You will ensure that adequate staff is available to provide services as needed, arrange substitutes when necessary, and provide direct care services as required.

As a resource to staff and families, you will assist in developing community connections and promote natural support systems for individuals served. You will participate in Person-Centered Planning efforts, advocating for the individuals to ensure they receive all necessary services.

Your duties will involve supervising DSPs, including training according to established protocols, ensuring consistent mentoring and support of individuals, and overseeing timekeeping and personnel matters. Additionally, you will assist in budgetary planning, implementing outings and programming for community integration, and advocating for necessary services.

You will be responsible for completing and/or monitoring progress reports on individuals, managing residents' finances, and reviewing and making changes to Waiver budgets as needed to ensure funding aligns with the services provided. Your commitment to the well-being and empowerment of individuals served will be central to your role as a Connection Leader.

Requirements

Preferred Minimum Qualifications
  • Individuals must be of unquestionable character as determined by personal references and background checks. Applicant must supply sufficient recommendations to support the fact that maturity, integrity, and dependability are hallmarks of their personality.
  • Preferably one full year of successful experience working with people with Intellectual and/or other types of Developmental Disabilities.
  • Must have a high school diploma or equivalent.
  • Must be able to effectively express oneself in written reports, including electronic documentation reporting.
  • Past supervisory experience desirable.
  • Must have good communication skills.
  • Ability to project a professional image while contacting community resources.
  • Must have a valid driver's license and acceptable driving record.
  • The ability to communicate effectively, both verbally and in writing, with all levels of co-workers to promote an effective teamwork approach to carrying out the responsibilities of the position, programs, and agency's goals.
  • Ability to physically restrain, lift, and/or assist clients.
  • Ability to occasionally climb stairs.
  • Must have the availability and willingness to take calls or assist in working as needed outside your normal scheduled hours.
  • Essential physical skills: be able to run, squat, stoop/bend, kneel, climb stairs, lift up to 50 pounds, push and pull against potential resistance.
