Job Description The **Antimicrobial Stewardship Pharmacist** will help to ensure appropriate use of antimicrobials at SBUH. The position provides input on medication regimens and protocols for the treatment of infectious diseases, primarily in inpatients. Concentration is on cost avoidance, optimization of antimicrobials and minimization of development of resistance to antimicrobials. Qualified candidates will demonstrate superior communication and organizational skills while adhering to our high standard of excellence. Develops policies and reviews treatment regimens involving antimicrobials. Develops and conducts MUEs involving the use of antimicrobial agents at SBUH . * Serves as co-chair of the Antimicrobial Stewardship Committee. Represents antimicrobial stewardship on the SBUH P&T Committee. Attends assigned clinical rounds, represents Pharmacy at other assigned committees.

* Reviews and verifies medication orders for patients on the assigned service. Reviews orders for appropriateness, suggests dose adjustments based on pharmacokinetic, laboratory or other data, suggests less expensive and/or more efficacious alternatives, coordinates dispensing/administration with Pharmacy when necessary.

* Provides educational programs for LIP, nurse, pharmacist staff, with a focus on optimal use of antimicrobial agents. Serves as preceptor for antimicrobial stewardship and ID rotations for the PGY-1 Pharmacy Practice Residency Program.

**Preferred Qualifications:** Completion of a Pharmacy Residency with a focus on infectious diseases and/or antimicrobial stewardship. *Advanced certification in infectious diseases. If candidate is ineligible for advanced certification, at the time of application, it is expected that the candidate will obtain advanced certification within 1 year of meeting eligibility requirements.* * Stony Brook Medicine is a smoke free environment.
