You are able to support and look at a patient holistically, helping them and their whole care team, navigate the health care system. Qualifications for Registered Nurse, RN Case Manager :A.D.N degree from accredited school of nursing required; B.S.N preferredCurrent Registered Nurse (RN) license in the state of practice requiredContinuing education credits maintained as required by state of practice requiredMinimum of three (3) years' experience in clinical nursing required, Renal Nursing a plusArmenian speaking preferredDemonstrated knowledge and understanding of data and managing to clinical, financial, and patient satisfaction outcomesExperience and passion for educating patientsCurrent CPR certification requiredAbility to adapt to new/changing technologyCurrent driver's license in state employed with positive driving record and able to meet requirements of insurance coverage requiredMust be located in the Glendale, CA areaHybrid position.
