Nurse (OPWDD - Residential) - RN or LPN - Military veterans preferred

A Long Island nonprofit services agency is looking to add a new, licensed Registered Nurse (RN) or Licensed Practice Nurse (LPN) to join their growing team on a Part-time basis at one of their residential facilities in Suffolk County. About the Opportunity: Setting: Residential Shift: Days Schedule: Full TimeMonday to Friday Hours: 7am to 3pm Patient Population: 11 to 21 year old individuals with ID/DD Responsibilities: The Residential Nurse will: Develop a person-centered plan of care that prescribes strategies and interventions Support discharge planning process with members of relevant teams Provide or coordinate the resident / family education Collect comprehensive data pertinent to the resident's health or the situation Perform all other duties as assigned Qualifications: 6+ months of related Nursing experience Graduate of an accredited School of Nursing NYS Registered Nurse (RN) license or LPN BLS certification Strong understanding of the needs of vulnerable populations including youth and young adults with Autism, Developmental Disabilities, Communication Impairments, and Medical ComplexitySolid assessment, clinical and documentation skills Patient oriented Great interpersonal skills Excellent communication skills (written and verbal) Strong attention to detail Highly organized Desired Skills: Bachelor's Degree in Nursing National Certification in area of specialty
