Job Title: Field Supervision Registered Nurse (RN)

Job Summary:
We are looking for an experienced Field Supervision Registered Nurse (RN) to join a large Home Care Agency team! As a Field RN Supervisor, you will make initial and follow-up patient and home assessment visits to determine service needs, develop a Plan of Care, instruct, and supervise paraprofessional staff. You will also set up and update patient's care records after every follow-up visit. This position requires travel to different boroughs in NYC.
Key Responsibilities:
Conduct comprehensive assessments of patients and families to determine home care needs
Develop and initiate plans of care with patients' physicians
Instruct, supervise and evaluate home health aide care
Perform follow-up visits for recertification, staff supervision, and changes in condition
Provide healthcare instructions to patients and families
Communicate with physicians regarding patient needs and changes in condition
Anticipate patient needs and recommend changes to physicians and families
Participate in clinical on-call coverage and respond actively and timely
Requirements:
* Current RN license in NY
* 2+ years of experience as a Registered or Practical Nurse, with 1+ year of field supervisory experience
* Baccalaureate degree in nursing or associate degree with supervisory experience
* Proof of vaccinations and negative TB test
* Malpractice insurance and CPR certification
