The Pharmacist is a patient oriented pharmacist whose objective is to improve the quality of patient care as related to drugs. He/she cooperates with the professional staff and within the policies and procedures of the Health Center, plans and participates in programs involving patient profiles, patient consultation on drug usage, drug information, facilitates monitoring chronic diseases, specialty clinics, monitoring patients as to drug effectiveness and function in in-service education programs for the medical, pharmacy and nursing staff and other support staff.1. Should develop a patient profile or drug record which should include all medications current and past and any known allergies or contradictions.2. Should develop a system for review of medications used to inform the patient of potential side effects, proper methods of taking medications and possible undesirable, but normal or expected reactions.3. Will provide specific drug information to other staff members as required or requested.4. Should develop a program to assist in the monitoring of patients on chronic therapy, e.g., diabetes, hypertension with respect to effectiveness of therapy and refilling of prescriptions. Compliance should also be monitored. The program should include primary screening for patients stabilized on chronic medications and refer to the proper clinics if anything appears to go wrong. He/she should not have sole responsibility of this program, but depend on nurses and physicians as circumstances indicate.5. Shall aid in the development and operation of specialty clinics which use standard medication, e.g., birth control, V.D., etc.6. Should be involved in special education programs for patients as to the types of drugs used, why and how they are used and the importance of following directions and the importance of home lab tests, e.g., urine, etc.7. Must complete Patient Safety Training curriculum.8. Perform additional duties as assigned.EDUCATION REQUIREMENTS:A degree from an accredited college of pharmacy.EXPERIENCE AND QUALIFICATIONS:1. Registered pharmacist in the State of Missouri.2. Experience in coordinating pharmacy programs and in teaching health professional students.3. Must have the ability to develop and coordinate a comprehensive health care program.4. Must have the ability to teach other health providers in all aspects of drug related patient care. Should meet academic qualifications to affiliate with educational facilities to train pharmacy students or support staff.5. Must have 1-2years experience working in a fast-paced pharmacy.6.Manager experience and 340B knowledge preferred.
