JOB SUMMARY

Responsible for the coordination of patient/family hospice care by direct supervision of the clinical team.Duties include hiring, evaluation, corrective action and separation of staff; coordination of staff schedules; staff orientation and education; management of resources within budgetary guidelines; active participation in the development of policies and procedures; participation in the development, implementation, and ongoing participation in the evaluation of the agency's Quality Assessment & Performance Improvement Program; and establishment of working relationships with internal and external customers. Clinical Manager of Hospice ensures quality end of life services to patient and family. Supportive of the Village's mission, values, and service/hospitality (PRIDE) expectations. Completes training as required by policy.

ESSENTIAL JOB FUNCTIONS

  • Responsible for ensuring compliance with all state and federal clinical home health regulations.
  • Conducts or ensures consistent, accurate and timely communication among all members of patient care team, families, patients, caregivers, service providers, etc.
  • Manages resources to ensure effective delivery of care within budget parameters.
  • Receives/coordinates referrals and screens for eligibility by telephone as appropriate.May review with the Director of Home Health/Hospice issues of appropriateness and eligibility for home health care/hospice. Refers to other service providers, as necessary.
  • Oversees intake process; coordinates patient assignment and delivery of patient care, assuring patient needs are continually assessed (home health)
  • Monitors and assesses the appropriateness and compliance with protocols, outcome assessments, formularies and third party payer requirements.
  • Responsible for orientation of new clinical staff nurses and Home Health Aides (CNA), including completion of department orientation checklist, and introduction/explanation of department policies and procedures.
  • Assures staff assignments are planned an completed as expected.
  • Assures licensing of clinical staff is current and other regulatory compliance factors for staff including required training and clinical competencies are met.
  • Evaluates the performance and competency of clinical and non-clinical staff direct reports.
  • Responsible to know home health/hospice regulations, to follow those regulations and to be prepared to meet with surveyors as needed.
  • Assures staff compliance with all agency requirements, local, state, and federal laws regarding licensure and certification of Village Home Health/Hospice associates.
  • Provides input into preparation of the annual budget for clinical services.
  • Ensures budgetary guidelines are being maintained in regard to purchase of supplies, medications, DME, etc.
  • Leads in the development, education, and administration of policies and procedures for Village Home Health/Hospice.
  • Responsible for development, implementation and updates of the individualized plan of care.
  • Facilitates a cooperative work group focused on teamwork and effective peer relationships.
  • Participates as needed with scheduled volunteer training sessions.
  • Responsible for coordination, implementation and evaluation of the care to the patients and supervises and monitors the patient care records.
  • Coordinates continuous care when appropriate (Hospice).
  • Ensures payer source(s) for clinical services.
  • Coordinates initial discharge from hospital, nursing home or other care settings into hospice care.
  • Coordinates admissions from home - either for acute care or respite care.
  • Identifies appropriate training opportunities for home health/hospice staff and schedules staff workload to permit attendance at these training sessions.
  • Reviews patient assessments and plans of care.
  • Tracks patient recertification and assures that they are performed.
  • Represents the John Knox Village Home Health/Hospice program to every constituency and public in a professional manner.
  • Adheres to all safety regulations and requirements.
  • Handles stress well and gets along with others within the workplace and with customers outside the department.
  • Maintains acceptable standards of workplace conduct.

ESSENTIAL QUALIFICATIONS

Education:
  • Bachelor's Degree in nursing, healthcare, or related field preferred, or a combination of education, training and/or experience that would typically be acquired through a course of study leading to graduating from an accredited school of professional nursing will be considered in lieu of bachelor's degree.
Licenses/Certifications:
  • RN licensed in MO and KS
  • A valid driver's license is required and proof of auto insurance.
  • Current Basic Life Support (BLS) certification or ability to obtain within 6 months of hire.
  • If Hospice, Hospice (CHPN) certification (must be obtained within 90 days of hire).
  • If Home Health, OASIS certification must be obtained within 90 days of hire
Knowledge and Skills (required unless otherwise noted):
  • Has expertise in pain management, symptom control, and other palliative clinical skills.
  • Knowledge of Medicare and clinical regulations. As well as other regulations and statutes governing the provision of healthcare in Missouri.
  • Must have knowledge of and exercise universal precautions and prevention of contamination.
  • Intermediate level of MS Word, Excel, and Outlook.Health MedX a plus.
  • Must be able to read, write, and communicate in English adequately to complete required reports, interpret Plan of Care instructions, and interact with the client/family, physicians, other caregivers, and staff.
  • Ability to exhibit professionalism, tact and patience when communicating with patients, clients, family's subordinate staff, staff at all levels and components of the organization, business associates, and the public.
  • Ability to interact effectively and empathetically with patients and their families.
  • Ability to plan and organize duties, including managing multiple tasks; ability to work under pressure with frequent interruptions; and ability to work independently, with a minimum amount of supervision.
  • Ability to communicate clearly and accurately, orally and in writing; ability to foster team interaction and ability to conduct efficient and effective meetings.
  • Ability to accurately and completely assess the client's current medical status and needs and to plan, implement and modify care plans as indicated.
  • Accepts responsibility for ensuring that the rights and dignity of patients, bereaved and their families are respected and safeguarded.
Experience:
  • Minimum of five (5) years clinical experience.
  • Minimum of two (2) years management/supervision experience in health care, including leadership of interdisciplinary team.
  • If Hospice, minimum of two (2) years Hospice experience.
SPECIAL WORKING CONDITIONS
  • The associate is subject frequently to inside environmental conditions with provide protection from weather conditions but not necessarily from temperature changes.
  • The associate is exposed to infectious diseases.
  • The associate is required to function around individual with challenged mental capacities.
  • Associate is required to drive from one location to another if necessary for services provided through the department.
  • The associate may be exposed to outdoor elements while accessing a patient's residence.
  • All associates may be called upon to assist other departments in a declared emergency situation.
