Champion Care is seeking a diligent and organized individual to fill the role of Case Manager Assistant. The primary objective of this position is to provide support to the case manager in coordinating and facilitating the progression of each case file. The Case Manager Assistant will collaborate closely with building Social Workers, Clinical Team Members, and Therapists, as well as with patients' assigned case managers provided through their insurance benefits.

This role entails executing specific duties in accordance with Champion Care's policies and procedures. The Case Manager Assistant will be responsible for various tasks under the guidance of the Case Manager.

Duties and Responsibilities:

  • Verify daily charting to ensure compliance with coverage guidelines.
  • Obtain necessary authorizations for therapy, coinsurance, room and board, medication, and treatment.
  • Effectively communicate with building team members regarding issuance of ABN's and NOMNCs.
  • Manage clinical data collection for timely insurance updates, including submission of service logs upon case closure.
  • Complete insurance appeals as required.
  • Ensure timely completion of Medicare Certifications and recertifications for each Medicare case file.
  • Review the content of patient case files at weekly Medicare meetings.
Qualifications:
  • Strong organizational and communication skills.
  • Proficient in medical terminology.
  • Minimum of 2 years of experience in long-term care.
Professional Requirements:
  • Adhere to dress code, maintain a neat and clean appearance.
  • Complete annual education requirements.
  • Maintain compliance with federal, state, and local regulations, as well as accrediting organization standards.
  • Ensure resident confidentiality is maintained at all times.
  • Report to work punctually and be willing to travel.
  • Wear identification while on duty.
  • Attend annual reviews and participate in departmental in-services.
  • Foster a positive and cooperative working relationship with physicians, departments, and staff.
  • Represent the organization in a positive and professional manner.
