Pharmacist 40hrs/Day/Evening Split ShiftPOSITION SUMMARY:Fill orders with accuracy; check technician prepared products before release; maintain security of pharmacy and controlled substances; enter orders and patient information with accuracy; aid in the training of new staff; update the Director about the daily activities of the pharmacy; aid the Director in maintaining the pharmacy. KEY RESPONSIBILITIES:Enters correct charges for all medicationsEnters correct medication, including description, strength, route, expiration date, and start/stop datesEnters mg/kg dosing for peds/neonates. Refers all pediatric orders for double checkCorrects all orders which are entered in error or may be misunderstood. Reviews and responds to all Holds by nursingPosts debits/credits accuratelyFollows all ATS conversions based on substitutions approved by P&TMaintains order entry error rate Enters all patient demographic information as neededCompletes work assignments within an acceptable time frameGenerates required reportsAlerts Director of any problems with system software or hardwareAssists technicians in performing technical tasksAccurately completes IV processMaintains aseptic techniqueRecycles products as appropriateLabels containers accurately including auxiliary labelsPrioritizes workloadAssists Tech in restocking shelves and rotating stockDisposes of all waste properlyChecks technician prepared products prior to release.Enters correct patient data for all TPN's via the intranetClarifies all TPN orders prior to order entryPerforms all calculations and worksheets for chemotherapyCompletes all documentation and double checksTakes required protective measures and ensures that technicians take all required protective measures.Disposes of chemotherapy waste properlyPrepares and dispenses drug products throughout the hospital (or directly supervises qualified pharmacy technician in these functions)Insures the appropriateness of drug dose and frequency for each medication ordered before dispensing. Clarifies any ambiguities with prescriber.Reviews all physician orders for appropriatenessPrepares, labels, and dispenses all drug products using appropriate techniqueFills all orders with correct medicationChecks support staff accuracy prior to medication deliveryPrioritizes work to prepare stat, now and waiting in order of importanceChecks all medications for delivery to nursing floors in a timely fashionOversees daily technician activities to maintain an effective workflowMaintains records as required by law to document drug transactions within the Pharmacy and throughout the hospital.Controlled drug sign-out sheets are initiated correctly and terminated correctly when new supplies vestigational drug logs and perpetual inventories are accurate and pletes all Controlled Substance logs and paperwork appropriatelyMaintains required Continuing education and gives copies to managementChecks that all batch logs are completed accuratelyFollows non-formulary drug proceduresCompletes medication error reports as neededOrders medications when stock is lowKeeps Director informed of abnormal use of itemsLoans and borrows medications per policySafely uses departmental equipmentDisposes of waste in the proper containersIs able to locate the MSDS Sheets, disaster manual, safety manual, Infection control, P&P, etc. (Use of hospital intranet)Demonstrates fire safety verbally.Checks expiration dates of all medicationsAssists in training of new staffHelps in other areas of pharmacy as neededIs able to refill, check and bill Emergency Med Carts, TPA kits, etcMaintains security of pharmacy.Maintains security of controlled substancesSupervises activities of support staff effectivelyProvides Director with feedback regarding activities of support staff.Work with other pharmacists and technicians to settle work priority disputesInforms oncoming staff of pending problems, call backs, etc Communication between shiftsReviews patient profile to ensure appropriate and safe drug therapyNo therapeutic duplication.Prevents significant drug interactions.Prevents avoidable ADR secondary to Allergies. Updates allergies on order entryMedication ordered is appropriate for Patient height, weight, age, renal function and disease state.Serves as a hospital resource for drug information, including indications, doses routes, side effects, adverse reactions, drug interactions, pharmacokinetics, and product availability. Able to utilize appropriate reference materials when necessary. TPN ScreeningContacts physician on any unusual order.Familiar with normal concentrations of additives.Capable of calculating fluid, electrolyte and calorie needsClarifies controversial orders with physician formulationParticipates in drug inventory control through formulary management, performing therapeutic interchanges per policy and purchasing.Responsible for maintaining the pharmaceutical quality assurance standardsProvides assistance to the Director of Pharmacy in attending specified. Committees and participating in activities of the hospital when requested.Provide input/suggestions to improve the pharmaceutical services provided.Is familiar with, and follows, Federal and State laws regarding the profession of pharmacy.Is knowledgeable in uses and dosages of medicines and is familiar with drug incompatibilities.Understands pharmaceutical and chemical procedures and techniques, and weights and measures in all systems.Understands the role of the pharmacy department in the hospital and it's relationship with other departments.Generates reports and completes all clinical functions related to reports such as Renal dosing, IV to PO, Falls Assessment and Anesthesia audits,Informs oncoming shift of patients or issues requiring monitoring or follow up.Reviews patient profile for requisite background information prior to contacting physician.Drug InformationKeep current on new drugsAnswers questions timely and accuratelyContinuously assesses and improves the performance of care and services provided. Offers creative solutions or alternatives to issues or concerns.Uses systematic methods to improve services, plan changes and evaluate outcomes.Demonstrates effective problem solving techniques and promotes same in others.Maintains awareness, actively supports the mission, goals and objective of the hospital and pletes special projects on time.Willingness to participate and complete other duties as assigned or requested.Is part of a team effort to insure quality and safe services.Produces quality results.Accepts responsibility for all work performed and takes appropriate corrective action as needed.Utilizes the pharmacy computer system to fulfill his/her job duties.Enters new drug orders into computer accurately. Checks all ordered drug against reported patient allergies, updates all allergies. Reviews existing patient drug therapy regarding potential drug interactions with proposed new drug REQUIRED KNOWLEDGE & SKILLS:Ability to work independently and handle stress appropriately. Ability to work cooperatively with other members of hospital and pharmacy staff. Ability to handle multiple tasks, and adapt to changes in workload and work schedule. Ability to set priorities, makes critical decisions, and responds to client concerns. Fosters a team environment. Ability to communicate effectively (verbal and written). EDUCATION/EXPERIENCE/LICENSURE/TECHNICAL/OTHER:Education: Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE). Doctor of Pharmacy degree preferred.Experience (Type & Length): One to two years of hospital pharmacy experience preferred. Hospital pharmacy residency preferred. Very good drug knowledge base including pharmacokinetics dosing and nutritional support. Working computer knowledge.
