Introduction \r GRADE 13\r LOCATION OF POSITION MDH Worcester County Health Department\r6040 Public Landing Road\rSnow Hill, MD 21863\r Main Purpose of Job The main purpose of this full-time contractual position will be to provide community-based case management to individuals with intellectual/developmental disabilities. This position will assist eligible individuals and families in gaining access to resources in their community, planning for their future, education on transition services and waivers that could be available to them, as well as accessing needed services and supports. This position will also work with the individual and their identified team members to create the individual's person-centered plan. In addition, this position will work with the DDA regional office, local providers, staff and/or other community partners regularly.\r\r\rThis position does include home visiting, community-based services and may require occasional evening and weekend hours.\r\r\rThe selected applicant must be able to work and travel to all Worcester County Health Department sites.\r\r\rPLEASE NOTE: There is a desired qualification per the COMAR regulations for this particular program - this position requires a Bachelor's degree from an accredited college or university in Nursing, Social Work, Psychology, Education, Counseling or a related field.\r MINIMUM QUALIFICATIONS Education: Graduation from an accredited high school or possession of a high school equivalency certificate. \rExperience: One year of administrative staff or professional work in a health care setting. \rNotes: \r1. Candidates may substitute 30 college credit hours from an accredited college or university for the required experience. \r2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education. \r DESIRED OR PREFERRED QUALIFICATIONS The desired candidate should possess a Bachelor’s degree from an accredited college or university in Nursing, Social Work, Psychology, Education, Counseling or a related field.\r SELECTION PROCESS Applicants who meet the minimum (and selective) qualifications will be included in further evaluation. The resulting certified eligible list for this recruitment may be used for similar positions in this or other State agencies.\r BENEFITS \r\rContractual employees who work for an agency covered under the State Employee and Retiree Health and Welfare Benefits Program, have a current employment contract and work 30 or more hours a week (or on average 130 hours per month) may be eligible for subsidized health benefits coverage for themselves and their dependents. As a contractual employee, you will be responsible for paying 25% of the premiums for your medical and prescription coverage, including any eligible dependents you have enrolled. The State of Maryland will subsidize the remaining 75% of the cost for these benefits. You can also elect to enroll in dental coverage, accidental death and dismemberment insurance, and life insurance, but will be responsible to pay the full premium for these benefits.\rLeave may be granted to a contractual employee who has worked 120 days in a 12 month period.
