Description
Position: Practice Medical Director - Florida
Practice Medical Directors are quality-driven physicians, who are dedicated to improving the well-being, quality of life, and health outcomes of our patients. We partner with the nation's leading clinical specialists to create transformative care. Monogram uses next-generation AI algorithms to predict necessary care, to promote the delay of complex disease progression, seamless transition to dialysis, preemptive kidney transplant, and conservative management. The PMD oversees the delivery of care management services within their market, ensuring high-quality patient care, by continually reviewing patient care plans and providing education and guidance to field clinicians when needed.
Responsibilities:
Responsible for large panels of health plan members in specified geographic regions in Florida
Utilize data tools to report, monitor, and improve quality and utilization
Conduct retrospective reviews of hospitalizations and dialysis treatments to ensure all services are effectively rendered
Oversee the implementation and refinement of clinical strategies, evidence-based guidelines and care management programs for effectiveness and efficiency
Reduce unnecessary episodes of care, including hospital readmissions
Lead case rounds with NPs and CMs
Analyze data to build member care plans and ensure care goals are achieved
Optimize patient health outcomes and total performance against health plan contracts
Represent Monogram as a leader to health plans, providers, and other organizations
Achieve clinical goals for delivering improved health outcomes at a lower cost of care
Requirements:
This is a hybrid telemedicine position with occasional in-home patient visits and some domestic travel
Demonstrated experience applying evidence based clinical criteria
Board Certified MD from accredited medical school with current Florida medical license willing to become licensed in multiple states
Experience with NCQA, HEDIS, Medicaid, Medicare, quality improvement, utilization management, and risk adjustment
Experience in CKD, ESRD, Chronic illness
