Legacy Hospice is looking for a Clinical Manager to join our Leadership Team in Lima, Ohio!

POSITION SUMMARY:

A Registered Nurse that assists the Administrator with thesupervision of hospice services to patients in their place of residence inaccordance with physician orders and strives to provide the highest quality ofcare. The ClinicalManager is available at all times during operatinghours (or appoints a similarly qualified alternate) and participates in allactivities relevant to professional services furnished, including thedevelopment of qualifications and assignment of personnel.

  • Registered Nurse with current licensure to practice in the state where hospice is located

RESPONSIBILITIES:
  • Supervise, orient and provide ongoing trainingfor clinical field teams
o Organizeand lead nursing meetings and in-service programs for field clinicians.

o Identifyneeds and provide additional training to clinical team based on clinicalaudits, outcome scores, reports and tandem visits. Reports received fromsoftware

o Trainingactivities and responsibilities include:

o Provideinitial Clinical Orientation for new field nursing staff.

o Coordinateand deliver training and testing

o TrainSupervisory visit nurses on proper assessment of the unskilled patient andappropriate referral to other services.

o Trainnurses in the field on admissions, skilled visits, and skilled procedures.

o Performtandem visits with nurses to provide support and training and to assesscompetency- Oasis, wound care, trach care, IV's, etc. Perform yearly and asneeded tandem competency visits and on site supervisory visits to supervisefield staff.

o Completeannual reviews and counseling of nursing staff.
  • Provide clinical support and direction to officeand field teams
o Providesupport to clinicians, office and field teams on clinical issues.

o Coverany cases that cannot be filled; back up clinical coverage.

o Ensureongoing communication with all members of the clinical team.
  • Provide clinical support and direction tomanagement and corporate teams and assure that overall daily clinical care issafe, appropriate and compliant with state, federal and nursing regulations andstandards
o Coordinatewith Coding and Clinical Review Nurse to assess cases identified as havingpotential inappropriate episode utilization or premature/inappropriate re-certor discharge.

o Coordinateand facilitate on site state and program specific compliance surveys' anddevelop plan of corrections as needed. Assure that plan of correction iscompleted.

o AssistAdministrators with coordination and implementation of QAPI.

o Collaboratewith entire Corporate Clinical Team and assist with onsite clinical followthru.

o Assistwith ADR's that require clinical intervention and documentation.

o Monitorreports in EHR software and review to assure proper utilization of skilledservices while still ensuring the patients receive proper and appropriate care.

o Monitorreports using EHR software and QAPI to improve data collection resulting inmore accurate reflection of client improvement, and to improve outcomesreported to the State by Oasis data collection.
  • Assure compliance with State, Federal and locallaws and Board of Nursing Regulations. Monitor and implement plans to achieveacceptable clinical outcomes. Assure compliance with Clinical Policy andProcedure
  • Perform audit/investigation of Adverse Eventsand report findings to Administrator
  • Performs data collection and assists with theAgency's ongoing Quality Assessment and Performance Improvement (QAPI) program
  • Oversee and maintain clinical supply utilizationand approve orders outside of threshold/protocol
  • Assists in the coordination of interdisciplinaryteam services, patient care, and referrals
  • Assures the development, implementation andupdates of the individualized patient plans of care
ADDITIONAL RESPONSIBILITIES:
  • The Clinical Manager is expected to keep theirCPR and RN licensure current
  • The Position is also expected to attendmandatory in-services and meetings, which may include travel
  • Supportive of individual and company performanceimprovement initiatives
  • Maintain confidentially with employee andcompany sensitive information
  • Comply with accepted professional standards andpractices
  • Complete projects and assignments as assigned byHospice Administrator
  • The Clinical Manager is expected to participatein appropriate continuing Education as required by the Agency
