The Chief Social Worker is also responsible for the social, emotional, and financial wellbeing of patients receiving services within the Behavioral Health Department. The Chief Social Worker will be considered part of the department's Leadership Team and participates in interdisciplinary treatment team planning; is responsible for discharge planning, running treatment groups as assigned, and performing appropriate psychosocial assessments and interventions. The Chief Social Worker will assist with referrals to the unit and make referrals to appropriate community agencies as indicated. KEY RESPONSIBILITIES Participates as a member of the Leadership Team for Behavioral Health ServicesProvides regular oversight and supervision of Social Worker employees and relevant professional studies students Develop an individualized plan of care with interdisciplinary treatment teamCoordinate discharge/follow-up activitiesDiscuss with patients/families the financial implications of placement decisions and provide guidanceProvide individualized psychotherapy, supportive counseling, and group therapy as indicatedMake referrals to community agencies as appropriateIdentify potential legal/risk management issues and review with Program DirectorParticipate in continuing education programs that are relevant to job functionDocument, in timely fashion, initial assessment, proposed plan, treatment updates, and final discharge in electronic medical recordCommunicate, in a timely manner, with staff, nursing facilities, agencies, and outpatient providersEducate patients/families, staff and providers regarding managed care and Medicare processes/regulationsMaintain current knowledge of DMH and applicable mental health legal status/advance directive regulatory requirementsProvide disease state teaching to patients/familiesProvide education/consultation to other healthcare team members as neededCollects statistical data as requested by supervisory personnelRegularly attends/participates in team/departmental meetings and activitiesAbides by and upholds the hospital's service excellence standards REQUIRED KNOWLEDGE & SKILLS:At least three years supervisory experienceStrong knowledge of DSM diagnoses and ICD-10Knowledge of medical terminologyExcellent organizational skills and ability to multi-task in a fast-paced environmentAbility to work both collaboratively and independentlyCase management skills EDUCATION/EXPERIENCE/LICENSURE/TECHNICAL/OTHER:Education:Master's Degree in Social WorkPreferred Experience:Five years in the clinical setting with a minimum one year's psychiatric case management experienceCertification / Licensure:Current MA licensure as an LICSW, with at least five years experience working as a LICSW Software / Hardware:Experience with Meditech preferredOther:N/ASteward Health Care is proud to be a minority, physician owned organization.
