Valeo Home Health and Hospice is seeking a?Hospice Assistant Clinical Manager !

As a?Assistant Clinical Manager,?you will be responsible for assisting the Director of Nursing with planning, direction, coordination and management as well as supervision of nursing professionals in conjunction with the Execuitve Director of Hospice. You will be responsible for assisting the Director of Nursing in reviewing the clinical performance of the nursing staff in all areas to help facilitate efficient patient care. You will help assure quality care by reviewing and assessing each service line in conjunction with the Director of Nursing. You will help maintain performance improvement activities within the organization and participate in continuous quality improvement activities.

Responsibilities:
    • Assess, supervise and direct patient care.
    • Oversee orientation and educational programs for field staff.
    • Perform patient care as required, including home assessments.
    • Develop plans of treatment.
    • Ensure and chart compliance with state regulations.
    • Community involvement including networking and marketing activities.
    • Evaluate and treat patients using the most current technology and practices.
    • Provide valuable input into the patient's care and treatment.

    • Preforms other duties as assigned.
Qualifications:
  • Licensed as a Registered Nurse by the State of Utah
  • Minimum 1 year of Hospice Supervisory or Management Experience
  • Experience with Homecare Homebase is preferred but not required
  • Ability to communicate effectively, both verbally and in writing
  • Respecting the patients privacy and maintaining confidentiality
  • Current Utah drivers license and automobile liability insurance
  • CPR Certification
  • Marketing or Sales experience a plus
