Description

UCFS is looking for candidates who are passionate about making a difference in the lives of others! We are currently seeking a highly motivated and experienced Nurse Manager to oversee our School-Based Health Centers. The Nurse Manager will be responsible for collaborateing with the Chief Operating Officer and Chief Medical Officer to proactively manage access to quality, patient-centered health care for UCFS's School Based Health Center Programs (SBHC). The Nurse Manager ensures appropriate staffing, workflows and support for multidisciplinary care teams to achieve positive patient health outcomes. As the cost center manager for the SBHC services, the Nurse Manager is responsible for departmental budgets and that UCFS meets or exceeds fiscal expectations. Initiates and maintains provider scheduling templates that meet or exceed productivity expectations.

Responsibilities:

  • Assures patient's access to care by maximizing provider schedules, consistent with standards of care, patient centered medical home principles and productivity expectations;
  • Monitors completion of documentation of patient encounters in accordance with timelines established in UCFS policy;
  • Represents SBHC in internal and external meetings, committees and initiatives to foster growth and improvement of services.
  • Support the consistent implementation of workflows, best and evidence based practices and standards of care resulting in improved quality metrics as determined by the organization, HRSA, CMS, NCQA PCMH, DSS and other entities toward the transition to value based care;
  • Develops and maintains centralized, accurate scheduling for primary care and behavioral health providers and their support staff that meets patient care needs including coverage arrangements for planned provider absences;

Requirements
  • Bachelor's degree in nursing (BSN) from an accredited institution
  • BS and 4 years of experience in clinical outpatient service experience, Associates and 7 years of experience.
  • 3 years of supervisory experience in a community health center preferred;
  • Experience working in an accredited Patient Centered Medical Home and or School Based Health Center practice preferred
  • CPR and First Aid certification
