How You'll Help Transform Healthcare:


Goal of the Rehab Liaison is to work in collaboration with the physicians and MLPs to identify clients/patients who would benefit from and meet the regulatory criteria for acute inpatient rehabilitation level of care. Work to facilitate effective care transition to the rehab setting for these clients, including but not limited to, admission screening, physical assessment, and completion of preadmission screening form (PAS). Uses clinical expertise and problem-solving skills to achieve optimal clinical and rehab outcomes within effective time frames. Promotes interdisciplinary collaboration, teamwork and champions service excellence. Must possess knowledge of growth and development appropriate to age group served and incorporate plan to meet needs into plan of care. Must also possess knowledge and skill of projection of rehab potential with/without physician input. Assess impact of admissions using UDS-PRO tools and identifies risks and alerts appropriate persons.

  • Early identification of candidates/potential clients.
  • Screening of clients through on site and/or telephonic assessment and record review in collaboration with physicians, nurses, therapists, social workers, case managers and other interdisciplinary team members involved in the client's care. If the client is unable to participate in an interview, family members or significant others may also be interviewed to obtain accurate information as to pre-morbid functional status, personal interests, cultural considerations, and discharge plans affecting the rehabilitation admission.
  • Education of clients, family members, referral sources, and external payers regarding the effective and efficient utilization of program services and available resources.
  • Determination of medical stability, readiness for program participation, and that the services recommended are appropriate, reasonable and necessary. This is done in collaboration with the physical medicine and rehabilitation physician or other licensed treating physician with specialized training and experience in rehabilitation.
  • Facilitation of benefit eligibility and precertification by payers prior to the rehabilitation admission.
  • Coordination of a smooth transition from the referring facility or site to the rehabilitation setting.
  • Recommendation of appropriate level of care and services for clients who are denied admission and providing guidance for clients who may benefit from rehabilitation admission in the future.
  • Identify, educate and provide professional presentations to potential referral sources in order to increase business growth.

What We Require:


Education: Registered Nurse, or Therapist (PT,OT,SLP) required; Bachelor's Degree or equivalent experience required.


Experience: Two years of recent experience in a rehab environment with responsibilities reflecting direct management of patient care including planning, coordination, and delivery of needed services such as education, psychosocial support, discharge planning and utilization management. Supervisory or leadership experience is preferred. Prefer 3-5 years experience direct patient care.


Licensure/Certifications/Registration: Current licensure in Virginia as a Registered Nurse, Physical Therapist, Occupational Therapist or Speech Language Pathologist. CRRN required within 24 months from date of hire as Rehab Liaison for registered nurses.
