Overview
Interprets and evaluates refill authorization requests for accuracy, completeness and appropriateness. Recognizes therapeutic problems including drug interactions, contraindications, improper dosing, and precautions of therapy. Recognizes actual or potential problems and provides therapeutic recommendations for their resolution.
Provides medication therapy management, medication reconciliation, allergy review and collaborates with providers to ensure safe and appropriate therapy. Appropriately reports all adverse drug reactions, drug defects, and medication errors.
Conducts therapeutic interchange under the collaborative drug therapy management program. May review authorization requests to identify and make recommendations on formulary alternatives that support cost effective prescribing. Collaborates with clinic and support staff to resolve authorization issues.
Participates in committees supporting refill authorization protocols and Epic development of tools to support workflows.
Assists in the development, maintenance and implementation of pharmacy programs to support overall services for patients.
Provides necessary support for service level agreements with ambulatory clinics
Job Related Knowledge, Skills and Abilities (Competencies):
Knowledge of pharmaceutical care principles.
Disease related modification of drug therapy.
Project management skills and experience in contermporary medication use systems.
Strong interpersonal communication skills and ability to work in a team setting.
Systematic problem solving skills.
Ability to work independently with minimal supervision.
Successfully completes initial and continued competencies as required for maintenance of standard of practice, legal regulatory compliance and OHSU policies
