The Field Case Management Supervisor role is part of Enlyte’s National Catastrophic Program. In this collaborative and crucial role, you will be managing complex cases and providing support to field case managers. You will

• Provide support and guidance to field case managers
• Handle catastrophic referral intake processes
• Oversee and monitor catastrophic cases
• Maintain effective communication with customers
• Serve as a resource for Field Case Managers, offering guidance and coaching
• Ensure adherence to account-specific guidelines
• Navigate complex medical cases and provide expert oversight in catastrophic case management
• Collaborate with team members and stakeholders
• Communicate effectively with various parties involved in case management
• Coach and mentor field case managers to improve their skills and performance
• Monitor and evaluate the progress of catastrophic cases
• Facilitate effective transitions of care for complex patients
