Job Summary
The Housing Coordinator is responsible for implementing the Ryan White Housing and Emergency Financial Assistance (EFA) services at the Bluegrass Care Clinic, ensuring alignment with program goals, local and federal regulations, and best practices. This position must have a high-level knowledge of local, state, and federal regulations and resources related to housing, as well as intensive case management experience. The Housing Coordinator conducts comprehensive housing assessments, enrolls eligible clients, and develops individualized housing stability plans, providing case management, home visits, and appointment support to help clients achieve housing stability and meet treatment goals.

The Housing Coordinator facilitates referrals to housing resources, educates clients on tenant rights, fair housing laws, and financial literacy, and assists with completing housing applications. In collaboration with the Basic Needs Team, this role works closely with landlords, utility companies, and the EFA Coordinator to process timely rent and utility payments while ensuring compliance with budgetary guidelines and funding requirements. The position maintains accurate documentation of client services in multiple databases, monitors housing trends, and provides recommendations for program improvements. Active participation in community coalitions, housing task forces, and partner agencies is essential to expanding housing opportunities for clients.

Additionally, the Housing Coordinator prepares reports for grant applications, conducts quality checks to meet program benchmarks, and delivers education on housing services to staff, clients, and community partners. Professionalism, teamwork, cultural competence and a client-centered approach are critical to ensuring effective service delivery and positive client outcomes.

Microsoft Windows, Excel, Comprehensive knowledge of Ryan White and Housing Programming, Written and Verbal Communication Skills, Case Management

MA in Sociology, Psychology, Human Development, Family Studies, and experience in the housing programs.
