Rehabilitation Case Manager (Neuro) - Military veterans preferred

For more than 35 years, we’ve developed programs to meet the diverse needs of clients with moderate to severe traumatic and acquired brain injuries. We provide treatment for post-concussion syndrome, stroke, memory loss, neurobehavioral issues and accident or sports-related head injuries.

Salary: $80,000 to $90,000 annually based on education and experience

We are searching for a Full-Time Rehabilitation Case Manager for our residential program located in Escondido, CA.

Essential Duties and Functions:

• Ensuring the implementation of the client's treatment plan(s), from admission through discharge and follow-up
• Coordinates schedules, therapies and other appointments/services
• Responsible for regular contact with families and funders
• Coordination with medical director and external medical providers to ensure appropriate medical care
• Collaborates with the program nurse and clinical director to ensure clinical and medical needs are being met.
• Communicates with the Operations Director to ensure residential care is being met and any personal preferences are identified.
• Report writing, clinical documentation and facilitating funding issues as needed
• Acts as a client advocate

Requirements:

• A bachelor’s degree along with case management in the healthcare industry/rehabilitation experience is required - a master’s degree and CRC or CCM designation is preferred
• Must have a valid driver's license and maintain responsible driving behaviors
