The Community Health Outreach & Enrollment Specialist enhances health literacy, expands healthcare access, and connects the underserved communities to vital resources. This role conducts outreach, provides health education, and screens patients for eligibility in programs like Medi-Cal, Covered California, FPACT, CHDP, Ryan White, and other financial assistance potions. The specialist also assists with enrollment, explains program benefits, and ensures patients receive the coverage needed for continued care. Working collaboratively with TrueCare departments and community partners, this role strengthens community connections and promotes long-term health access.

Duties & Responsibilities:

  • Coordinate and attend community events (health fairs, food distributions, flu shot events, forums) to promote TrueCare services, provide health awareness (i.e. blood pressure screenings), enroll new patients, and increase visits.
  • Distributes informational materials outlining available TrueCare services, locations, and schedules.
  • Conduct presentations for Spanish-speaking, low-income, and medically indigent populations in North San Diego and Riverside County.
  • Partner with community organizations to deliver culturally relevant health information on key topics (e.g., substance use, nutrition, dental health).
  • Maintain a database of community-based resources to address social determinants of health and build rapport with community members.
  • Screen and assist patients with enrollment in health coverage programs, including Medi-Cal, Covered California, FPACT, CHDP, Ryan White, and other episodic coverage options.
  • Educate patients on program benefits and ensure compliance with federal regulations regarding Medi-Cal/Covered California coaching.
  • Assist with completing and processing applications, ensuring timely follow-up for enrollment confirmation and renewals.
  • Track and report retention efforts, case load data, and newly insured patient referrals.
  •  Actively promote Medi-Cal and Covered California both internally and externally.
  • Identify new referral sources to increase enrollment of newly insured patients.
  • Work closely with front desk staff to coordinate and schedule patient appointments.
  • Refer patients to other TrueCare services as appropriate.
  • Perform basic health assessments, including blood pressure screenings, height, and weight measurements.
  • Maintain accurate documentation of outreach activities, enrollment efforts, and referrals.
  • Manage department inventory and ensure materials are available for outreach and patient education.
  • Follow appointment schedules and communicate any delays to supervisors as needed.
  • Participate in department meetings, training sessions, and quality improvement activities.
  • Assist in data collection and reporting for program evaluations and grant requirements. 
  • Support the development of community presentations, lesson plans, and educational materials in collaboration with internal and external experts.
  • Adapt to staffing needs by working as an interim PRS at various locations.
  • Maintain professionalism by providing exceptional customer service, promptly answering patient questions, and ensuring a welcoming environment.

Required Qualifications:

  • Associate degree in healthcare or related field or equivalent work experience.
  • At least two (2) years’ experience in health care or related field performing outreach, education, case work, and/or customer service to diverse populations.
  • Knowledge of local community-based agencies.
  • Availability to work regularly outside of normal shifts (i.e., 8am to 5pm).
  • Public speaking, computer literacy, including internet navigation and research skills, and proficiency with the Microsoft Office suite, including Outlook, Word, Excel, and PowerPoint.
  • Bilingual in English and Spanish is required.

The pay range for this role is $23 - $32.20 on an hourly basis.

If you are hired at TrueCare, your final base salary (within the pay range), will be determined based on factors such as education, knowledge, skills, and experience. In addition to those factors, we believe in the importance of pay equity and consider the internal equity of our current team members when determining any final offer. Please keep in mind that the range mentioned above is the full base salary range for the role. Hiring at the maximum of the range would not be typical to allow for future and continued salary growth.
