Job Summary

This position will offer remote work flexibility, but the individual hired for this role must reside in the state of Ohio. 

Responsible to develop effective partnership and engagement strategies with internal and external stakeholders and oversee pilot programs to test and deploy innovative models for Population Health Integration and Designs across the state health plan. Responsible for developing and driving all aspects of the strategic plan that focuses on member equity inclusive of major health care concerns and social determinants of health, including but not limited to food insecurities, housing, employment, and incarceration repatriation. Responsible for developing and implementing a multi-faceted program dedicated to evaluating member success and program outcomes. Interfaces with local and national business partners, customers, and regulatory entities. Will design pilots and partnership agreements that further the goals of innovative methods to deliver value-based care for MHI. Direct and guide program and project staff in implementation and evaluation of programs and operationalizing processes. Responsible for designing strategy and implementing the comprehensive Population Health operations and integration strategy that supports and delivers on Molina’s vision, values, goals, operation, and financial performance.

 

Job Duties

  • Oversee Molina’s state health plan strategic design, implementation, and evaluation of population health initiatives based on a deep understanding of scientific population health principles
  • Sponsor and champion state health plan’s system-wide initiatives, including cultivating the support necessary to achieve the desired operational objectives for each initiative
  • Liaison with the state’s Department of Medicaid, the Single Pharmacy Benefit Manager (SPBM), and other Medicaid-contracted health plans on population health initiatives; and 
  • develop and implement operational plans that address the market opportunities / challenges and align with established population health goals. 
  • Designs / assists with the design of all aspects of Population Health Program.
  • Builds out analytics and reporting to support the Population Health Program.
  • Develops scorecards, tools, and processes to drive implementation and evaluation of the Population Health Programs.
  • Leverages population health knowledge and initiative implementation skills to work effectively with clinical and operational stakeholders to synthesize program information, develop and deploy measurement tools and oversee timelines, budgets, and ROI analysis.
  • Leads meetings and committees, directs the data analysis and reporting, monitors program performance and looks ahead to mitigate risks.
  • Creates culture and linguistic program description following NCQA accreditation 
  • standards.
  • Creates health education programs that support food insecurity, housing, employment, and incarceration repatriation.
  • Provides leadership and strategic direction to integrate operational assets to support population health infrastructure needs across the state health plan.
  • Partners with Clinical Quality Performance Management leadership to develop market-appropriate roadmaps, tools, and playbooks.
  • Fosters a culture of data-driven improvement.
  • Develops policies, procedures, and guidelines for population health operations programs.
  • Evaluates regulatory changes and determine steps needed to implement in programs.
  • Identifies business development opportunities to support growth of population health programs 
  • Oversees and navigates technology and software applications utilized in the delivery of population health programs.
  • Identifies delivery systems utilized in member care for population health.

 

Job Qualifications

REQUIRED EDUCATION:

  • Bachelor’s Degree in nursing, social work, health services research, health policy, information technology, or other relevant field.

REQUIRED EXPERIENCE/KNOWLEDGE, SKILLS & ABILITIES:

  • 5 or more years of progressive experience in population health, service coordination, ambulatory care, long-term care, community public health, case or care management, or coordinating care across multiple settings and with multiple providers. 
  • Development and execution of evidence-based population health management integration programs.

PREFERRED EDUCATION:

Master’s Degree or other advanced degree in nursing, social work, health services research, health policy, information technology, or other relevant field.

Pay Range: $79,607.91 - $172,483.8 / ANNUAL
