Population Health Clinical Specialist

The Population Health Clinical Specialist is focused on improving health outcomes and addressing health disparities of the patient population at Access through collaboration with the Population Health team. The position works to improve rates of evidence-based preventive screenings and support self-management of chronic disease through outreach efforts, case management, and innovative approaches to patient care between scheduled visits.

Desired candidate would have the following qualifications:

Required Qualifications:

  • Licensed Practical Nurse (LPN)
  • Medical Assistant Training (MA)
  • Certified Medical Assistant (CMA) required within a year of hire

Desired Qualifications:
  • One year or more experience as LPN/CMA
  • Previous work experience in medical primary clinic setting
  • Fluency in English and Spanish
