Case Manager Mill Brook Terrace Older Adult Center

 

The Case Manager will work as part of the Mill Brook Terrace Older Adult Center team.  They are a non-exempt employee.  A candidate with a Bachelors’ Degree is preferred; a candidate with a high school diploma and a minimum of five years’ experience working with seniors would be considered.

 

The Case Manager reports to, and is closely supervised, by the Director of the Mill Brook Terrace Older Adult Center.  The Case Manager will provide a comprehensive range of case management services to the members of the Older Adult Center.  

Responsibilities


  • Assist members in obtaining and maintaining appropriate medical and mental health services.
  • Assist members in obtaining and maintaining entitlements.
  • Assist members in obtaining and maintaining home care.
  • Provide care coordination, as needed, between community service providers.
  • Assist members in establishing and/or maintaining family and social relationships.
  • Through in-house activities and community referrals, assist members in participating in socialization opportunities.
  • Assist members with budgeting, income maintenance etc.     
  • Provide crisis intervention services as needed.
  • Provide recreational group work services for members of the Older Adult Center.
  • Complete non-reportable Incident Reports.
  • Maintain professional roles and boundaries.
  • Maintain thorough case records, including case notes within the time frames required by WSFSSH and the contract/regulatory agencies where applicable.
  • Work cooperatively with other staff members.
  • Share information about Older Adult Center members progress, needs, and problems with other staff as appropriate, guided by the WSFSSH Confidentiality Policy.  
  • Develop and maintain competency in software and data bases as required.
  • Attend training sessions and conferences as required for enhancement of job skills.
  • Implement emergency procedures as necessary.
  • Attend supervision meetings, Older Adult Center team meetings and Mill Brook Terrace all staff meetings.
  • Maintain awareness of and sensitivity to issues of Diversity, Equity and Inclusion.
  • Other duties as assigned.

Qualifications

  • BSW preferred; BA in human services field with at least 2 years case management experience will be considered; HS Diploma with at least 5 years of case management experience will be considered
  • Fluency in written and oral English required
  • Bi-lingual Spanish a plus
  • Experience with older adults and people with mental illness and/or substance use issues preferred
