Position Highlights

  • $31.56-$39.66/hour DOE with the ability to go higher for highly experienced candidates

  • Additional pay for your bilingual skills

  • $5,000.00 Hiring/Retention Bonus Structure:

    • At Hire: $1,500.00

    • At 90-Days (3 months): $1,500.00

    • At 180-Days (6 months): $2,000.00

    • Bonus will be pro-rated based on Full Time Equivalency (FTE)

What You'll Do:

  • Obtains complete history and conducts chart review, ensuring inclusion of all reports needed.
  • Performs baseline physical assessment and expands assessment based on initial findings.
  • Collaborates with health care Providers in developing and implementing plan of care.
  • Coordinates and plans interagency referrals as directed by the Provider or Registered Nurse (RN).
  • Assists Providers in diagnostic procedures and treatments.
  • Provides education to patient and family/caregivers taking into consideration age, development, culture, language, and resources available.
  • Coordinates plan of care and other resources.
  • Evaluates care based on assessments and Physician orders, and initiates follow-up care plan.
  • Assists in managing medications based on purpose, side effects and drug interactions.
  • Collaborates with ancillary staff to process patients based on Providers’ schedule.
  • Oversees subordinate nursing staffs’ provision of patient care.
  • Plans and organizes clinical supplies, equipment and furnishings to maintain a safe patient environment.
  • Performs other duties as assigned.
  • Performs clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), and YVFWC requirements.

Qualifications:

  • Graduation from an accredited Licensed Practical Nurse (LPN) program.
  • One year's experience as an LPN preferred.
  • LPN License for the state of practice.
  • Cardiopulmonary Resuscitation (CPR) certification within 90 days of hire.
  • Bilingual (English/Spanish) preferred. Must demonstrate the ability to communicate at level 10 on the language proficiency scale to receive bilingual differential pay.
  • Knowledge of established professional LPN concepts and principles.
  • Knowledge of the uses and effects of medications.
  • Knowledge of how to access local community agencies and resources.
  • Effective verbal, written and listening communication skills.
  • Strong attention to detail and high level of accuracy.
  • Ability to understand, assess and react effectively to the unique needs of patients.
  • Ability to prioritize work, handle a variety of tasks simultaneously in a fast-paced environment.
  • Ability to effectively collaborate with medical professionals in a team framework.
  • Basic proficiency with a variety of computer programs including Microsoft Outlook, Word, and Excel.
  • Proficiency with Electronic Medical Records (EMR) preferred.
