We are searching for a Rehabilitation Case Manager in our residential program located in Raleigh, NC. This position is responsible for ensuring the implementation of the clients’ treatment plan(s), from admission through discharge and follow-up. This role coordinates schedules, therapies and other appointments/services, and is responsible for regular contact with families and funders, as well as ongoing internal report writing, clinical documentation and facilitating funding issues as needed. The individual serving in this position acts as a client advocate and is responsible for client treatment reports and chart documentation.


Duties:

  • Leads team meetings and interfaces with treatment team to establish rehabilitation goals and coordinates treatment necessary to support client’s treatment plan
  • Responsible for monitoring the clients’ treatment plan and progress towards goals; provides problem solving, conflict resolution and revision of goals and treatment plan as necessary
  • Meets with Clinical Specialist/Director regularly to update and review client’s plan, progress, goals and schedule
  • In consultation with the clinical team, identifies, collaborates, and consults with physicians and other health care providers outside of the primary treatment team
  • Works directly with the clinical team and direct care staff to design systems or strategies necessary to support the clients’ treatment plans

Requirements:

  • A Bachelor’s degree along with case management in the healthcare industry/rehabilitation experience is
    required
  • Master’s degree and CRC or CCM designation is preferred
  • Must have a valid driver's license and maintain responsible driving behaviors
