Job Summary:

Case Manager/Housing Specialist is responsible for providing specialized Case Management Services to individuals with a primary diagnosis of Severe and Persistent Mental Illness (SMI) who are transitioning from institutional or congregate settings to community-based housing. The goal is to maximize independence through Community Integrated Housing with the outcome of increased community tenure, as identified in the PCRP (Person Centered Recovery Plan).

Essential Functions:

• Provide housing resource development, dissemination of information and assistance for securing housing for individuals identified under DIVERTS Services, (Direct Intervention Vital Early Response Treatment System), and those identified by In Reach Coordinator transitioning to the community as part of Amended Settlement Agreements).
• Coordinates and facilitates housing applications, screenings, lease signings, and move in process in conjunction with the In Reach Coordinator and ACT Team.
• Provides Case Management Services to those with Olmstead Vouchers for thirty-six months post move in and assists in transition to permanent voucher programs if the person qualifies. Conducts regular meetings and check-ins to assess person’s progress, address challenges and provide guidance and problem-solving assistance.
• Provides Supportive Housing services in accordance with SAMHSA Permanent Supportive Housing Toolkit, conducts an annual self-assessment of supportive housing services using the SAMSHA Permanent Supportive Housing Fidelity Scale, as required by BHDID contract.
• May provide short term Targeted Case Management Services to those from our Region prior to date of discharge from Western State Hosptial and post discharge. Routine visits to WSH may be planned and coordinated with VP of Community Services and In Reach Coordinator.
• Participates and attends State Housing Meetings-Olmstead Meetings as required. Participates in ACT, In Reach, TCM and Supported Employment weekly meetings at the agency.
• Other duties as assigned.

Education and Experience:

• Bachelors degree in a behavioral science from an accredited institution. Behavioral sciences include Psychology, Social Work, Sociology, Human Services, Special Education, Family Studies, Counseling, and Nursing.
• One (1) year experience in case management or working with the chronically mentally ill population after Bachelor's degree in a behavioral science is completed. A Masters degree in a behavioral science can substitute for the 1 year of experience.
• Valid driver’s license and reliable transportation.
• Good oral and written communication skills.
• Physical ability to do light lifting.
• State Case Management certification within six (6) months of employment.
• All case managers are required to complete and pass the Safe Crisis Management 2-day class prior to independent functioning. The SCM training must be re-certified annually.
