JOB SUMMARY
The Parent Navigator will advocate, mentor, engage, empower and support families to take the lead in their own Plan of Care and helps connect families to needed resources within their own communities. Services are in-home and/or within the community. Parent Navigators will help families navigate services and assistance from community resources through putting families in touch with available services.
KEY RESPONSIBILITIES
Services provided to families are based on individual needs of the family, but may include:
Providing assessment of family needs
Providing advocacy and support for children and families with medical coverage, mental health, parenting education, substance abuse and schools
Enhancing parent's capacity and ability to cope and manage stress related to a variety of issues including but limited to: family history of abuse and/or neglect, physical and mental health problems, marital conflict, parenting concerns, substance abuse, domestic violence, community violence, unemployment, limited or insufficient financial resources, difficulty managing resources, unstable housing, isolation, special needs of their children, child development, safety and immunizations
Assisting with relationship management within the family and/or family’s system of care and support
Identifying and utilizing community resources and supports to meet the needs of the family
Referring families to support programs and services to enhance family functioning to enhance family functioning
Empowering families through education and support to enable them to take the lead role in planning for and responding to their family's needs
Assisting families in meeting their basic needs
Complete and maintain all required documentation of client records, home visits, assessments, etc. in required time frames
Maintain a collaborative relationship with primary care providers and community services
Experience and Skills
EDUCATION
High school diploma with experience in parenting skills and community services
EXPERIENCE
Bilingual - Spanish preferred
Must have a current valid state issued Driver License and own a vehicle for home visits
Good communication skills both verbal and written
Solid general computer skills a plus
Physically able to perform the essential functions of the position, with or without reasonable accommodations
