Job Overview

The Case Manager Biomedical Program Infectious Disease (ID) functions as an integral member of the Biomedical team, responsible for tasks facilitating the client’s ability to obtain biomedical interventions that will support them to engage in and remain in medical care. The Case Manager is responsible for patient advocacy, applying for public benefits and insurance coverage, and supporting clients to eliminate barriers to obtaining care. Other duties may include a range of settings based on where high-risk patients are identified.

• Minimum Requirements

  • High school diploma.

  • Bachelor’s degree in Human Services or equivalent work experience within a related field of Health and Human Services.

  • Los Angeles County HIV Case Management Certification.

  • A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

  • Bilingual English/Spanish required.

Compensation

$26.91 - $33.53 hourly
